Instructions for Completing the Application for Admission

1. Request an application by mail:

   Requests can also be made by phone at (419) 520-2600, by e-mail at admissions@medcentral.edu, or you can download an application in Adobe Acrobat format.
2. Submit official transcripts from high school and all postsecondary institutions attended. Official transcripts must be sent directly to MedCentral from the academic institutions.
3. Return the completed application to MedCentral College of Nursing, along with the nonrefundable $50 application fee, payable to MedCentral College of Nursing.
4. Arrange to take the ACT or SAT test if you graduated from high school within the
   last three years. MedCentral College of Nursing Codes:
• ACT: 3292
• SAT: 3935
5. The Admission and Progression Committee may request a personal interview when all information has been received.
6. All required admission materials must be received before an application will be considered by the Admission and Progression Committee. Submit all required admission materials by February 1. Admission is competitive. The admission decision will be communicated by letter.
